This application form is is to be used for 2019 membership running from 1 January 2019 to 31 December 2019. Existing members should renew their membership between 2nd November and 31st December 2018.

New members should contact the Membership Secretary for any new applications prior to using this form.This can be done in person or through the Contact Page of the web site.

Membership covers Club Membership, ASA membership and Central Lancs subscriptions for the year up to 31st December 2019. 

If you have any problems with the application process, please raise a Contact query and we will be more than happy to assist you with the application.

On completion of the form, please make payment of your Membership Fee and notify the club that you have made payment by completing the Membership Payment Form on this page http://www.oldhamseals.org/membership-payment-confirmation/

In addition, please ensure your monthly subscriptions are in line with the relevant monthly fees.

 

NOTES :

The information within this form shall be held by Oldham Seals ASC until 28th February 2020 and then deleted and replaced by information provided in subsequent membership applications for the following year.

Any current 2018 members not rejoining in 2019 will have their data removed from the Membership system on 28th February 2019.

If a member leaves the club they can request removal of their information on leaving through the Contact form and, following confirmation of no outstanding fees due, the information will be removed from the membership club records.

The information provided shall be used by the following committee members for the following reasons.

Web Administrator : Collection of the data on the web site and distributed where required to the following committee members.

Membership Secretary : ALL of the supplied information : For the administration of club membership, for the liaison with the ASA for membership.

Assistant Membership Secretary : ALL of the supplied information : For the administration of club membership, for the liaison with the ASA for membership.

Treasurer : Limited information as necessary : For the management of Membership Fees and Monthly Subscriptions

Competition Secretary : Limited information as necessary : To manage the organisation and administration of competition applications.

Welfare Officer : Parent / Guardian contact information : Only in the case of a welfare concern.

The club shall normally provide information through the website Subscription service, through social media and occasionally by post.
